On October 13, 2016, an employee was troubleshooting a squealing roller on a conveyor. The guard was removed and the employee tapped on the roller with a hammer. The roller caught the hammer and pulled his finger into it. He was transferred to the hospital where the left index finger was amputated at the first joint.

An employee was moving a piece of pipe on a cart when it shifted and crushed his right-hand middle and index fingers, fracturing both. The index finger above the first knuckle had to be surgically amputated.
